Sprout Family Health is reimagining healthcare for families with young children. Our approach goes beyond traditional reactive care: we blend medical expertise with lifestyle guidance to create tailored, development-focused care plans that fit seamlessly into daily life, prioritizing illness prevention and maximizing whole-family wellness. Sprout offers flexible, integrated care crafted to grow with each family member’s unique needs. At Sprout, we believe that fostering great health should be as warm, personal, and joyful as family life itself.
As the Patient Care and Operations Lead at Sprout Family Health, you’ll play a pivotal role in our mission to provide flexible, individualized care for families, working collaboratively with our team of healthcare professionals to ensure the smooth operation of our family-centered practice. Your expertise will be crucial in managing patient flow, coordinating appointments, and delivering exceptional service that enhances patient experiences, all while addressing the unique needs of each family member. This position is full of room for growth, allowing you to implement and improve processes that enhance patient care and operational efficiency, and make a meaningful impact. If you are passionate about providing exceptional and proactive healthcare and love the idea of doing family healthcare the way it should be done, we would love to hear from you!
In this role, you’ll be responsible for providing warm and professional patient greetings, managing inquiries via phone, email, and in-person channels, and ensuring seamless patient support in collaboration with our Health Navigator. You will handle clinic operations by overseeing scheduling for the multidisciplinary team, managing electronic health records, coordinating diagnostic tests and referrals, and conducting meet-and-greet calls for new members. As a leader, you’ll train and mentor team members, collaborate with clinical staff to improve patient flow and clinic efficiency, and contribute to policy and procedure development. You’ll also coordinate special services, manage medical supply inventory, support new service implementation, and work closely with the marketing team.
